What to report

Please report any of the following (facts only):

  1. Late ballot delivery / missing ballots
  • Not received at all
  • Received too late to return by mail
  • Returned mail or address issues
  1. Different packet contents
  • Different print versions (color vs. black-and-white)
  • Missing candidate bio(s) or missing pages
  • Missing return materials (outer envelope/return envelope)
  1. Replacement ballot irregularities
  • You requested a replacement and were denied
  • You were given a ballot in an unusual manner
  • Anyone opened a returned packet or handled ballots improperly
  1. Pressure or solicitation
  • Requests to sign over a proxy/designated voter form
  • Contact pressuring your vote or demanding proxy sign-over
  • Intimidation related to petition signing or voting
  1. Ballot handling / custody concerns
  • You were prevented from self-delivering a sealed ballot
  • Ballots were “handled for you”
  • Unclear storage/custody before counting

How to report (confidential)

Subject line: ELECTION ISSUE – Unit ___

Please include:

  • Unit number
  • In-state / out-of-state
  • Ballot received date (or “not received as of ___”)
  • What happened (3–6 bullets, facts only)
  • Who you contacted (management/board/monitor) and when
  • Any proof you have (photos/screenshots/tracking)

If you are uncomfortable including your name in writing, you may submit as:

“Owner – Unit ___ (confidential)”

(Identity can be provided to regulators off-record if required.)

Preserve originals (this matters)

If you still have election materials, do not discard them. Preserve:

  • Outer mailing envelope (front/back)
  • Any UPS/USPS/FedEx labels and tracking pages
  • Ballot packet contents (keep together)
  • Any return envelope(s)
  • Any voting certificate/designated voter forms you received
  • Screenshots of emails/texts regarding ballots/proxies

Do not mark, rewrite, or “correct” documents. Keep them intact.

Photo checklist (safe, useful, minimal)

If you photograph materials, focus on:

  1. Outer envelope front showing postmark/tracking
  2. Outer envelope back showing stamps/labels
  3. One photo of all packet contents laid out (one shot)
  4. Any email/text showing instructions, denial, or pressure

Avoid photographing your ballot selections. Focus on envelopes, labels, dates, and missing materials.

Optional “sworn-style” mini statement starter

If you want to submit a statement that’s easily convertible into a declaration:

I am the owner of Unit ____. I received my ballot on ____ (or have not received it as of ____). The issue I experienced was: ____. I contacted ____ on ____ and received the following response: ____. I have preserved the original materials and can provide them if requested.

Privacy and retaliation concerns

If you are worried about retaliation, say so in your email. You may submit:

  • unit number only, or
  • unit number + a safe callback method, or
  • unit number + request for an off-record ID process
